Follow these steps to enroll in Florida's self-exclusion program.

Open the Seminole Tribe of Florida form from PlayersEdge and choose one year, five years, or lifetime

Enroll in person with Security at a Seminole casino, or complete the form for enrollment by mail

For mail enrollment, sign before a notary and include copies of valid federal or state identification and a current photograph

Mail the completed documents to the Security Department at the nearest Seminole casino
- 5
Contact any non-Seminole cardroom, slot facility, or casino directly to ask about its separate self-exclusion process
Ending or Removing Self-Exclusion
One- and five-year exclusions expire automatically and cannot be appealed early. After at least five years, a person with a lifetime exclusion may submit a written termination request to the Seminole Tribal Gaming Commission. The official form does not say that a request guarantees removal.
Important Notes
The Florida Gaming Control Commission does not currently publish a single voluntary form or list that covers both Seminole and non-Seminole gambling. Its guidance says legal venues must provide self-exclusion options. The Seminole program is separate, so confirm the process directly with each non-Seminole venue you need to block.
