Arizona uses two separate statewide self-exclusion lists. One covers Indian gaming facilities and their associated services. The other covers licensed retail and mobile event wagering and licensed fantasy sports contests. You can complete either form or both, and each form offers a one-year, five-year, or ten-year term.

Choose the casino application, the event-wagering and fantasy-sports application, or both. Each program has its own form.

Open the Arizona Department of Gaming self-exclusion page and choose the casino form, the event-wagering and fantasy-sports form, or both

Complete each selected form in full and choose a one-year, five-year, or ten-year term

Have the form notarized and include the current color photograph required by the Department

Submit a scanned PDF and photograph by email, mail the documents to the address on the form, or arrange an in-person appointment at the Department's Phoenix office

Call 602-771-4263 or email the self-exclusion team shown on the official page if you need to confirm receipt, processing, or which form covers a specific product
Ending or Removing Self-Exclusion
The selected one-year, five-year, or ten-year term is irrevocable and cannot be shortened, altered, or rescinded. The Department says your name is removed only after the full term ends, and it does not send an expiration notice. To continue after expiration, submit a new notarized form and current color photograph.
Important Notes
Arizona maintains separate casino and event-wagering/fantasy-sports exclusions. Completing one does not place you on the other list, although you may complete both.
