Follow these steps to enroll in Louisiana's self-exclusion program.

Open the current Louisiana State Police instructions and request form and read the program terms before traveling

Choose a Louisiana State Police Gaming Enforcement Division field office from the current instructions and confirm its hours before traveling

Appear in person with valid government-issued identification; a Division agent must witness the request and take your photograph

Complete and sign the state request with the Division agent. The request is not submitted at a casino

For a faster account-level pause, a sportsbook must also offer its own online self-restriction for at least 30 days; that operator control is separate from joining the state list
Ending or Removing Self-Exclusion
Your name remains on the state list after five years. Removal is not automatic. You must submit a written request stating specific reasons, include the required recommendation from a qualified mental health professional, attend a nonpublic hearing, and receive a final decision approving removal. A licensee may continue to deny gaming privileges after state-list removal.
Important Notes
The verified state process does not support a separate lifetime choice. Current rules require state-list enforcement at regulated casino gaming establishments and sports wagering platforms. They do not support blanket claims that the list covers every video draw poker location, the Louisiana Lottery, tribal gaming, fantasy sports, unlicensed sites, or gambling outside Louisiana. Operator sportsbook self-restriction is a separate account-level control.
