Follow these steps to enroll in Connecticut's self-exclusion program.

Choose a one-year, five-year, or lifetime exclusion

Provide the identifying information requested by the application and agree to its terms

Wait for the Department's confirmation email before relying on the exclusion; the official form says registration is not active until that confirmation arrives

Use the separate Mohegan Sun or Foxwoods form linked by the state if you also want exclusion from gaming conducted solely on a tribal reservation

Ending or Removing Self-Exclusion
One-year and five-year exclusions do not end automatically. After the selected period expires, you must request removal through the state portal. A lifetime exclusion cannot be removed.
Important Notes
One state request covers internet and in-person gambling offered by Connecticut Gaming Licensees, including fantasy contests. It does not cover internet or other gaming conducted solely on a tribal reservation. Use the separate tribal program forms linked by the Department of Consumer Protection for that coverage. A licensee may apply stricter or broader exclusions to affiliated properties and products.
