How to Apply

Follow these steps to enroll in Illinois's self-exclusion program.

    Review the current program rules and authorized enrollment locations on the Illinois Gaming Board website

    Schedule or visit an authorized enrollment site; enrollment must be completed in person

  3. 3

    Bring valid government-issued photo identification

  4. 4

    Complete the application, acknowledgments, and required interview with an authorized agent

  5. 5

    Have the required photograph taken and submit the signed application

Ending or Removing Self-Exclusion

The exclusion does not end automatically. After at least five years, you may apply to the Illinois Gaming Board for removal. The application requires treatment documentation and a recommendation from a qualified professional. The IGB reviews the request and may require further investigation or Board action.

Important Notes

The Illinois Gaming Board describes this program as covering casinos and sports wagering. Video gaming terminal exclusion is not listed as part of this statewide program.
