Follow these steps to enroll in Michigan's self-exclusion program.

Choose the Disassociated Persons List for Detroit casinos or the Responsible Gaming Database for authorized online gaming and sports betting

Download the current form and instructions from the Michigan Gaming Control Board

For the Disassociated Persons List, arrange an in-person appointment and bring valid government-issued photo identification

For the Responsible Gaming Database, select internet gaming, internet sports betting, or both and choose a one-year or five-year term

Submit the Responsible Gaming Database form in person, by mail, or by email with the required notarization and identification

Contact each tribal casino separately because Michigan does not operate one statewide tribal-casino exclusion list
Ending or Removing Self-Exclusion
Responsible Gaming Database exclusions end automatically when the selected one-year or five-year term expires. A person on the Disassociated Persons List may request removal after at least five years; removal is not automatic.
Important Notes
Michigan uses separate state programs for Detroit commercial casinos and state-authorized online gambling. Tribal casinos run their own exclusion processes.
