Follow these steps to enroll in New York's self-exclusion program.

Open the current Voluntary Self-Exclusion form on the New York State Gaming Commission website

Choose a term of one year, three years, five years, or lifetime and complete the requested identity information

Include a recent photograph that meets the instructions on the form

Submit at a licensed gaming facility's designated problem gambling intake area, at Commission headquarters during business hours, or by U.S. mail

If submitting by mail, have the form notarized and send it with the photograph to the Commission address shown on the form
Ending or Removing Self-Exclusion
A one-, three-, or five-year term cannot be shortened. The Commission removes the person when that term ends unless a new form with an updated photo is submitted within 30 days before expiration to extend it. A lifetime term has no scheduled removal under the current rule.
Important Notes
The current form applies to gaming facilities and entities licensed, permitted, or registered by the New York State Gaming Commission. It does not list tribal casinos, so confirm self-exclusion separately with any tribal property you need to cover.
