How to Apply

Follow these steps to enroll in New Jersey's self-exclusion program.

    Visit the NJ Division of Gaming Enforcement website

    Download the Self-Exclusion Request form

    Complete the form with your personal information

    Choose exclusion period: 1 year, 5 years, or lifetime

    Submit in person at the DGE office in Atlantic City or Trenton

    Bring valid government-issued photo ID

  7. 7

    Your photograph and signature will be recorded

How to Reverse Self-Exclusion
