Follow these steps to enroll in West Virginia's self-exclusion program.

Open the current West Virginia Lottery Self-Exclusion Request Form and read every acknowledgement before completing it

Complete every required field, read and initial each acknowledgement, and attach a copy of the government-issued photo identification described by the form

Complete the form in the presence of West Virginia Lottery Security Personnel, or have the signature notarized when completing it elsewhere
- 4
Mail the completed form to the West Virginia Lottery address printed on the current form, or contact the Lottery before arranging an in-person submission
- 5
Use the separate responsible-gaming control inside the iPlay account if you also want to exclude from account-based lottery services
Ending or Removing Self-Exclusion
The statewide exclusion remains in place permanently unless the West Virginia Lottery Commission or a court determines otherwise. The Commission will consider a written removal request submitted on its current removal form after at least one year, but approval is not automatic.
Important Notes
The statewide form covers casinos, iGaming, and sports betting associated with the West Virginia Lottery. It explicitly does not exclude a person from the Lottery's iPlay account; that requires a separate self-exclusion through the iPlay website, app, or support center. The official form also states that inclusion on the statewide list is a public record.
