Follow these steps to enroll in Colorado's self-exclusion program.

Open the Colorado Division of Gaming self-exclusion page and choose the online application or downloadable PDF

Choose a one-year, three-year, or five-year exclusion period
- 3
Complete every requested field and include the identification documents required by the application
- 4
Submit the online form, or send the completed PDF by email, mail, or drop-off to the Division of Gaming
- 5
Keep a copy of the submission and check with the Division if you need confirmation that it has been processed

Ending or Removing Self-Exclusion
Removal is not automatic when the selected period ends. You remain on the list until you request removal and the Colorado Division of Gaming Director approves it. You may petition before the selected period ends, but the Director has full discretion, may require an evaluation, and does not guarantee approval. After a denial, the published rule requires waiting one year before submitting another petition.
Important Notes
The state list covers gaming regulated by the Colorado Limited Gaming Control Commission and Division of Gaming. Do not assume that it covers the Colorado Lottery, tribal gaming, horse racing, fantasy contests, gambling outside Colorado, or products outside those agencies' authority. Colorado fantasy contest operators maintain their own self-exclusion processes. An operator may also apply a broader corporate exclusion after informing you.
