Follow these steps to enroll in Iowa's self-exclusion program.

Choose a five-year or lifetime exclusion

Choose mail, an appointment at the IRGC office in Des Moines, an Iowa Gambling Treatment Program agency, or a state-licensed casino or racetrack

For mail enrollment, include the completed form, a copy of current photo ID, a recent head-and-shoulders photo, and a notarized signature

For an IRGC office appointment, bring current government-issued photo ID and complete the form with a representative

For a treatment agency, bring current photo ID and a recent head-and-shoulders photo; complete notarization there when available or before mailing the form, photo, and ID copy

For a licensed casino or racetrack, bring current government-issued photo ID and complete the form with a facility representative

Allow up to two weeks for all licensed operators to update their systems after the term begins

Ending or Removing Self-Exclusion
A five-year exclusion ends automatically five years after the enrollment form was signed and cannot be ended early. A lifetime exclusion signed on or after July 1, 2017 cannot be ended. Older lifetime enrollments follow a separate revocation process described by the IRGC.
Important Notes
The statewide program covers operators licensed by the Iowa Racing and Gaming Commission. The term starts on the date next to the signature, although licensed operators may take up to two weeks to update their systems. It bars gambling with licensed operators and entry to the gaming floor or wagering area of a licensed casino or racetrack. It does not automatically prohibit hotels, restaurants, convention centers, or other non-gambling areas, but each facility defines its restricted areas. A facility may also maintain a separate corporate exclusion.
