How to Apply

Follow these steps to enroll in Ohio's self-exclusion program.

    Choose a minimum one-year, minimum five-year, or lifetime exclusion

    Apply through the online portal or at an Ohio casino or video-lottery facility

  4. 4

    Provide the required identity information, photograph, and acknowledgments

  5. 5

    Contact the Ohio Casino Control Commission if you need an alternative enrollment arrangement

Ending or Removing Self-Exclusion

Removal is not automatic. After the selected minimum period, you may request removal under the regulator's process. A person who selected lifetime exclusion may apply after at least five years and must complete the required problem-gambling education before removal can be considered.
