Oklahoma has two separate verified routes. The Oklahoma Association on Problem Gambling and Gaming processes one form for participating tribal gaming commissions and casinos, but not every Oklahoma casino participates. The Oklahoma Lottery has a separate lifetime agreement. Complete both if you need both kinds of gambling covered.
How to Apply for Oklahoma Self-Exclusion
Use the OAPGG route for its current participating casinos and the separate Lottery agreement for Oklahoma Lottery products.

Open OAPGG's self-exclusion page and check the live participating-tribe list before assuming a casino is covered

For the multi-casino route, submit the online form and complete OAPGG's brief online notary session, or use the downloadable form and sign it before a notary

Attach a copy of your current driver's license and a separate recent photograph; choose one year, three years, five years, or ten years

For Oklahoma Lottery products, complete the separate lifetime agreement, attach the required photo and state-issued ID copy, have it notarized, and submit it using the agreement's instructions

The OAPGG request is irrevocable for the selected term. Its form directs a person to contact each gaming facility about removal after that term. The Oklahoma Lottery agreement is a lifetime exclusion and the current rules do not publish an individual early-removal route.

OAPGG describes its program as statewide, but its current page and form say participation is voluntary and not every tribe participates. Treat the live participant list as the coverage boundary. The Lottery agreement is separate and says retail purchase enforcement is not currently feasible; its practical controls focus on prize claims over $600, drawings, player-club access, and marketing.
