The New Mexico Gaming Control Board lets you request exclusion from one or more named licensed gaming facilities. Its current form can cover selected racetrack casinos, all racetrack casinos, or gaming facilities statewide, including racetrack and tribal gaming facilities. Applications must be completed in person with Board staff; they cannot be mailed.
How to Apply for New Mexico Self-Exclusion
Schedule an in-person meeting with the Gaming Control Board and choose the facilities and term shown on the current form.

Open the Gaming Control Board self-exclusion page and review the current request form before choosing a term or facility scope

Call 505-841-9700 or use the Board's scheduling page to arrange an in-person meeting at 4900 Alameda Boulevard NE in Albuquerque or ask about another approved location

Bring a government-issued photo ID with your signature and photograph; Board staff will take a current photograph during the appointment

Choose a one-year, five-year, lifetime, or other available term and identify the racetrack or tribal gaming facilities you want covered

Complete and submit the request with Board staff; do not mail it because the Board requires in-person identity verification

A fixed term remains in effect for the full selected period and the form says the name is removed when that period expires. A person may also file a verified written petition for removal, but the Board will not consider it until at least one year has passed. The petitioner must show that removal is in the public interest, and the Board says it will decide within 90 days.

Coverage follows the facilities identified in the approved request. The Board says exclusion applies to gaming operations and gaming-floor areas at those facilities. A tribal gaming authority may impose a broader property restriction.
